Dr. Leonoff’s office is located off of Rt 58 in Riverhead. Commerce Drive is located between Panera Bread and Harley Davidson, across from Riverhead Toyota and PC Richard. Building 54 is in the Northwest Corner of Commerce. Suite 8 is around the building on the Southwest corner. Just follow the signs when you get to building 54. Please note that Google Maps come up as Calverton but our mailing address is Riverhead.

Insurance and Billing
We accept most traditional insurance plans, contact our office to verify
acceptance of your plan. David R. Leonoff, DDS, PC does not participate in
Health Management Organizations (HMO/DMO), Medicaid or Community Plan.
